After you have read the above (and below) directions and instructions, please schedule your appointment, by clicking on the blue "NEXT" button. Be sure that you receive the final screen which states that your appointment was succesful. If not, "refresh" your screen and try again (somebody else may have taken this appointment before you finished submitting online).
